Carpal tunnel surgery is a procedure used for removing the stress of daily life on the hands. It is also known as median Sternenbund (SS) surgery and carpal disarticulation.
Carpal tunnel syndrome is a chronic pain condition that happens in the wrist when you rotate your hand toward your thumb. It occurs when two opposing forces, such as squeezing and un-squeeging, occur at the same time.
When un-squeezing happens, it can cause problems such as misalignment of hand and wrist or damage to nerves that connect the palm to the rest of the body. The reverse occurs when a person squeezes, with resulting pain and discomfort.
This type of surgery can be done on either side of the wrist, making it an accessible surgery for most people. Because it can be done overnight by some, it is a good choice for patients who need another surgery day or need another after this one has healing properties.

General anesthesia
Most surgery is done in a general anesthesia state. This means that you can think about nothing but the situation you are in and your response to it. You will be too distracted to give any thought to your pain or recovery, except for during the surgery.
However, since this is a state, there are some precautions taken. First, there is an epidural device placed near the spine. Then, there is an anesthetic being used. Lastly, there is a IV administration of an Anesthesia flowing into your body.
Because of all of these things, it is hard to predict what kind of pain you will feel while in the surgery state and what effects Anesthesia will have on you.
General anesthesia has two sides: completely side-less and completely side-full.
Local anesthetic
When medicine does not work, it can sometimes be due to an incorrect choice of medication or technique. This can often result in the patient suffering and/or losing a limb. Knowing which type of medicine is used for ear, nose, and throat surgery is a important part of this knowledge.
Local anesthetic is one of the many types of medicine. This type of medicine does not put you to sleep, but rather makes you less awake by using an anesthesia. When used for surgery, local anesthetic makes it easier to achieve and maintain surgical safety and effectiveness.
Since the patient cannot feel any sensations during surgery, it is easier to determine if the surgery was successful because the patient feels no difference in feeling or appearance. It also reduces the risk of postoperative pain which can hinder recovery.
Intravenous (IV) sedation
Intravenous (IV) sedation is used as a main anesthetic, or additional anesthetic, for surgery that requires anesthesia. During surgery, the patient is hooked up to a IV with a drip and general anesthetic is administered.
During surgery, the patient is monitored by anesthesia personnel via multiple monitors. Some of these include: A video screen, computer monitor, phone screen, and bedside monitor.
General anesthetics like ivpromobitol or meprobion are typical examples of general anesthetics that are used as extra anesthesia for surgeries that require extra bedside manner.
Conscious sedation
Another method of anesthesia is conscious sedation. This can be done with a combination of ketamine, midodine, and diazepam. These medications are typically not used alone, but in conjunction with other drugs.
These drugs can be very unpleasant to drink, so it is recommended that the patient be fully awake and aware during the procedure. Some patients do not feel comfortable being fully awake during the procedure due to feeling overwhelmed or being too Mentally stable (Mtb) for the procedure.
By having the patients be fully asleep during the procedure and using only minimal medications, there is less risk of side effects or difficultly managing if any side effects occur. This is possible because of the sedation effect of ketamine and midodine.
Needle apophysioanal blockade (NAB)
A less invasive method of anesthesia is called needle apophysioanal blockade (NAB). This technique involves placing a large, brightly colored needle under the skin near the wrist.
This prevents any pain medication from reaching the nerve that sends pain to the wrist. It also reduces stimulation to the nerves that convey pain, making it more likely that they will relax and allow blood and lymph to flow, which reduces pain.
Because NAB does not involve surgery or analgesics, most doctor do not require specialty training in order for them to use NAB for surgery. As with any new method of anesthetic delivery, there are always some mistakes being made.

Carpal tunnel release surgery
The most common type of surgery for the wrist is carpal tunnel surgery. This procedure affects the nerves that run down the middle of the hand and into the thumb. These nerves are removed, and a small steel plate and metallic wire is placed in its place.
This operation can be performed on both hands, but typically it is done to remove the nerve that runs down the middle of the thumb and into the forefinger. This nerve is removed to prevent any re-growth or return of tissue that may have collapsed during healing.
Re-instituting this nerve has a chance of causing new problems, such as returning pain or numbness in areas that have been healed. Therefore, professional doctors can tell if a person has enough strength in their own arms and hands to do this operation!

What to expect post-op
After a carpal tunnel surgery, your surgeon will typically leave you in a supine position for about an hour. This allows him or her to come back and check on you to make sure it worked!
You will be immobilized throughout the surgery and removed to your feet where you must stand on your own. You will probably have some pain as you stand on your own, but it will go away shortly.
Some patients feel dizzy or disoriented for a short while after their surgery, but that usually pass. Your reflexes may be slow post-surgery, so stay patient and active.
